First Reconciliation/First Holy Communion Registration Form - 2025/2026

This form is for registering children in Grade 2 or above (as of September 2025) to prepare for the Sacraments of First Reconciliation and First Holy Communion.

To register for this program, please ensure your
- family is registered parishioners and/or lives within the boundary of St. Jerome Parish.
- candidate has received the Sacrament of Baptism.

If any of the above are not met, please contact sjp_sacraments@archtoronto.org prior to completing this registration form.

Registration Requirements
(1) Complete and submit the Registration/Application form online.
(2) Upload an original or certified true copy of the child's ROMAN CATHOLIC baptismal certificate, including the Parish's complete address.
(3) Choose your preferred class (Saturday morning or Tuesday evening).
(4) Select a date and time for the Parent Information Session (PIS) with the pastor.
(5) Pay the online registration fee of C$100.

Note: Application approval depends on meeting all requirements.

There are 12 mandatory in-person sessions, at the church, held on Saturdays or Tuesdays, starting in November 2025, January 2026, and February 2026.

✅Parents are expected to supervise and authenticate the completion of all their children’s home assignments.
The family is encouraged to attend the weekend Masses.

Please complete the payment online using debit or any major credit card. A $100 payment is collected to help defray the costs of administering the formation program which includes all materials.
